Referral for periodontal and endodontic services

Our clinic is privileged to host Dr Deon Naicker, a specialist endodontist, during his visits to Darwin. Referrals to Dr Naicker can be made using the electronic referral form below.

Our periodontal team includes Drs Diaz and Huang. Please, note on your referral whether you would like for your patient to be seen by a specific clinician. Otherwise, we will endeavour for your patient to be seen as soon as possible by the next available clinician.

HOW TO REFER

There are two ways of referring your patients for periodontal OR endodontic treatment:

1. Call our office on (08) 8981 9993 during business hours (Mon-Fri 8am-5pm, Sat 08.30-12pm).

2. Complete the referral form below.

Appointments will be available on Monday through to Friday, between 08.30am and 4pm.

If you wish to send existing radiographs, please email them to contact@darwinperiodontics.com.au or ask your patient to bring them to the consultation, if they are in hard copy. Alternately, you can upload them to the secure folder the link to which your practice has received from us.

After referral

Following initial consultation and relevant clinical information gathering, a report of the findings and any recommended treatment will be mailed or (where an email address is provided) emailed to you. Our clinic also utilises a HIPAA-compliant folder structure for secure sharing of information with our referring clinicians.

General dental treatment

If any general dental treatment is required prior to or during specialist care, your patient will be asked to return to you for all necessary general dental procedures. Our specialists will contact you electronically and by phone with details of the recommended treatment.

After treatment

Following completion of active specialist treatment, your patient will be asked to return to you for general dental care. A treatment report will be mailed or emailed to you, including recommendations for any required ongoing care requirements. 

Supportive Periodontal Therapy

If your patient requires ongoing periodontal maintenance, Darwin Periodontics will liaise with you to determine whether you are able to offer this service to your patient or if you prefer Darwin Periodontics to continue patient care in this aspect.
